Electronic Referral and Privacy Forum Feedback

The forum on electronic referal and privacy took place at the Ashley Ricketson Centre onTuesday 19th August 2008.  The forum highlighted the need to ensure a balance between controlling personal information and ensuring that the right information is available to the right people at the right time in the right way.  It was attended by over 40 people.  A full report on the forum can be found on the InfoXange web site using the following link:  http://www.s2s.org.au/news/

Privacy Resources for Primary Care Agencies

All non government (private sector) health and community care providers are responsible under both Commonwealth privacy legislation and the Victorian Health Records Act for health and other sensitive information (racial origins, religious beliefs, sexual preferences, criminal record etc). The Health Privacy Principles should be used as the primary guide to compliance.

Although both the Department of Human Services and Primary Care Partnerships provide support and advice to agencies, the primary responsibility for privacy compliance rests with each agency.

ABHI (Australian Better Health Initiative)

In June 2008 Monash and Southcity GP Divisions commenced a 3 year service integration project to improve health care for people with chronic diseases under the Australian Better Health Initiative (ABHI). A work plan has been developed and the initial focus is on diabetes management. ISEPICH is a member of the Project Steering Committee and this project is providing an opportunity to identify and work with the Divisions on service coordination issues in the inner south east.

Service Coordination and Integrated Chronic Disease Management

ISEPICH has brought Service Coordination and Integrated Chronic Disease Management together through the SCIDM advisory group. The plan will be updated annually or as required.

The three key themes of ISEPICH'S SCIDM Action Plan 2008 8 seconds download via 56K modemSCIDM 2008 Action Plan are:

  1. Expanding the number of agencies demonstrating integrated Service Coordination practices, especially the use of the SCTTs;
  2. Promoting the electronic transmission of client referrals and care plans;
  3. Promoting integrated chronic disease management (incorporating a health promotion approach), self-management of chronic disease and client participation;

ISEPICH Chronic Disease Management Forum October 2007

ISEPICH held a forum on integrated chronic disease management on 11 October 2007. The aims of the forum were:

  1. To commence collaborative strategic planning for integrated chronic disease management in the ISEPICH catchment.
  2. To share information on current chronic disease management and self-management support in the catchment.
  3. To review and discuss the DHS process for self-management mapping and agree what additional information we wish to collect locally.

Victorian Service Coordination Practice Manual

The Victorian Service Coordination Practice Manual defines the common practices, processes, protocols and systems which support Service Coordination across Victoria. It replaces all local and regional Service Coordination Practices, Processes, Protocols and Systems (PPPS) manuals.
