New SCIDM project Coordinator
ISEPICH has appointed a new Service Coordination and Integrated Chronic Disease Management (SCIDM) Project Coordinator, Jillian Ramsden, in September 2011. Jillian is currently working with key stakeholders to update SCIDM action plans.
Elder Abuse Project
Australian research indicates that between 1 and 5 percent of senior Victorians may be experiencing some form of harm or abuse. The Victorian Government has funded an Elder Abuse Prevention Strategy in the Southern Metropolitan Region. The following resources and support are available for agencies and community members:
- Community Information Sessions and Discussion Workshops
- Staff Information Sessions and Training Workshops
- Agency resources, approved by the Victorian Government: Policy, Interagency Response Framework, Elder Abuse Prevention Guide (these resources are templates which may be adapted to suit individual agency circumstances. A Publisher version of the Guide in is available for agencies to modify as required.)

SCIDM
ISEPICH has brought Service Coordination and Integrated Chronic Disease Management together through the SCIDM advisory group. For more information about Service Coordination and Integrated Chronic Disease Management in ISEPICH, see the ISEPICH_SCIDM_Operational_Plan_and_Implementation_2009-12. Revised Action Plans for 2011-12 are currently being developed.

Carer Profile

Commonwealth Carer Respite Services has asked that agencies in the SMR ISEPICH use a Carer Profile for referral into the Commonwealth Carer Respite Services. This Profile is currently being used in regions across Victoria and provides an interim solution for Carer Respite Services awaiting the development of state wide Carer Profile as part of SCTT. Privacy Resources for Primary Care Agencies
All non government (private sector) health and community care providers are responsible under both Commonwealth privacy legislation and the Victorian Health Records Act for health and other sensitive information (racial origins, religious beliefs, sexual preferences, criminal record etc). The Health Privacy Principles should be used as the primary guide to compliance.
Although both the Department of Human Services and Primary Care Partnerships provide support and advice to agencies, the primary responsibility for privacy compliance rests with each agency.

The Victorian Service Coordination Practice Manual defines the common practices, processes, protocols and systems which support Service Coordination across Victoria. It replaces all local and regional Service Coordination Practices, Processes, Protocols and Systems (PPPS) manuals.

Tool Templates (SCoTTs)
To support service coordination, the Department of Human Services has developed common forms, called "service coordination templates" (SCoTTs), to standardise the collection of information by primary care agencies.

Self Paced Training Module
A self-paced training module for staff involved in service coordination activities is now available. 'Service Coordination: What? Why? How?' is available in 3 formats:

- On CD; and
- On a CD that can be customised by PCPs and agencies.
The module has been developed to complement agency orientation and induction programs.
