The Integrated Health Promotion Plan
One of ISEPICH's key aims is to promote the health and wellbeing of the community.
ISEPICH member agencies and community representatives work together to develop, implement and evaluate an Integrated Health Promotion Plan (IHPP). ISEPICH has three priorities for integrated health promotion in 2006-09:
- mental wellbeing
- physical activity
- nutrition and food security

Below is a summary of key goals and objectives and yearly Action Plans for each health promotion priority area.

Mental wellbeing
The ISEPICH Mental Wellbeing Promotion Plan
The 2007-09 plan is available here ISEPICH Mental Wellbeing Promotion Plan 2007-09
Goal:
To improve mental wellbeing in the ISEPICH catchment (the cities of Glen Eira, Port Phillip and Stonnington).
Objectives:
The ISEPICH Mental Wellbeing Action Plan is an action research plan, which is progressively updated as each stage is completed.
A key objective in 2007 was to increase the shared understanding of mental wellbeing promotion in ISEPICH. We held a workshop on mental wellbeing promotion, attended by 35 people, including representatives of nineteen agencies and community representatives.
A Mental Wellbeing Action Group (MWAG) was set up, and developed a vision statement based on key principles identified at the forum ISEPICH mental wellbeing promotion vision 2007.

The objective for the first half of 2008 is to develop an overview of what is currently being done to promote mental wellbeing in ISEPICH, and what it is achieving, with a focus on outcomes for participants and community.
Based on the results of this work we will then identify further activities for 2008-09.
The MWAG will also continue to support ongoing ISEPICH projects, including those described below.
A Local Response to Welfare to Work - consultation and advocacy project
Port Phillip Community Group, in partnership with local agencies, is collecting stories about the impacts of 'Welfare to Work' from workers and people affected by the new system.

Agreed approach to Health Promotion in Rooming Houses
ISEPICH member agencies have a long standing interest in working with Rooming House residents and managers to improve wellbeing and living conditions. In 2002-03 the Partnership provided some funding to support a settings-based health promotion project in a local Rooming House.
Residents who participated in the project reported a greater sense of cohesion, a positive response to new problem solving mechanisms and improved access to services.

In 2006 Resolve Community Consulting conducted a further project to look at health and wellbeing needs in several local Rooming Houses.

Building the capacity of ISEPICH agencies to provide inclusive health promotion and services

Advocacy to increase affordable housing in the catchment
ISEPICH is developing strategies to support the provision of more affordable housing.
The ongoing partnership initiatives in mental wellbeing promotion will also contribute to the development of a health inequalities framework for the partnership.

Physical activity
Goal:
To improve the health of the local community through increased physical activity levels.
Objectives (2008):
- To increase capacity for integrated physical activity promotion in the partnership, including an improved funding base, more integrated planning, improved links with GPs, and increased community participation.
- To increase the reach of physical activity initiatives to key population groups, including older people, children, culturally and linguistically diverse groups, disadvantaged groups and people with disabilities.
- To increase the capacity of the partnership to address the environmental determinants of physical activity.

ISEPICH will also continue to support ongoing partnership initiatives, including those described below.
Out There and Active
This project is funded 'Go for your life Seniors'. It aims to increase walking amonst older adults through a pedometer loan scheme and improved environments for walking. For more information see Out There and Active - HALS project plan
Stepping Out
This project aims to increase the capacity for walking groups in the area, particularly for disadvantaged groups, people with disabilities and culturally and linguistically diverse groups, through recruiting volunteer walk leaders and walk companions.
It is funded through 'Go for your life' Walk Together grants and commences in October 2007. A project plan is available here Stepping out summary project plan
Falls Prevention Community Network
The Falls Prevention Community Network has been active for over six years and supports sustainable falls prevention in the area. ISEPICH in partnership with Kingston Bayside PCP has also received funding from the Department of Human Services Aged Care branch for the 'No Falls' project.
In 2007 this project reached hundreds of local citizens through peer education sessions in English and other languages, and provided awareness training to over 50 workers from 21 local organisations.

- The DHS Health Promotion website has evidence based resources on falls prevention, and also a resource on cardiovascular disease and diabetes type 2, which contains a section on physical activity. The resources are available at www.health.vic.gov.au/healthpromotion/quality/evidence_index.htm
- A Physical Activity Resource from Sport England - this web based resource has been developed for health partners through the Primary Care Trusts, to support them with planning, delivering and measuring physical activity. - TravelSmart is a joint initiative of the Australian, State and Territory Governments to reduce reliance on cars and encourage other forms of transport, including walking and cycling. - Arthritis Victoria offers Physical Activity and Exercise Leader Training Courses for health and fitness professionals. Courses include Tai Chi for Arthritis, Warm Water Exercise, Active Chair Based Exercise and Strength Training. Nutrition & Food Security
Food security may be defined as ”the state in which all persons obtain a nutritionally adequate, culturally acceptable diet at all times through local non-emergency sources" (Food Security in the City of Port Phillip Report Part V 2005 p.2)
The Nutrition and Food Security Action Group conducted a survey of ISEPICH member agencies on food insecurity in 2006. A summary of results is available Summary results from 2006 survey
Goal:
To improve access to healthy and affordable food in the catchment, particularly for disadvantaged people.
Objectives (2008):
- To provide a framework that enables ISEPICH member agencies to work collaboratively to promote nutrition and food security
- To support the local provision of emergency relief, meals, nutrition education and skill development to individuals, families and population groups that are experiencing food insecurity
- To increase capacity to address the social determinants of food security and nutrition in ISEPICH.

Evaluation
The evaluation resource 'Planning for Effective Health Promotion Evaluation May 2005' can be downloaded from the DHS Health Promotion website at www.health.vic.gov.au/healthpromotion
The Department of Human Services funded an 'Evaluation Skills Development in the Primary Care Sector' project in 2004-05.I SEPICH participated in the regional project and is developing a catchment evaluation framework. ISEPICH plans and reports are also progressively being added to the QIPPS database at http://qipps.cs.latrobe.edu.au/ and will be made available on the public QIPPS library when completed.
