Community Care Services
The City of Port Phillip provides a range of Community Care services to meet the needs of younger people with a disability and frail older people and their carers maintain their independence and quality of life.
Eligibility for Services
Eligibility for community care services is restricted to frail older people, people with disabilities and their carers.
Access to Community Care Services
You can call us your self, or a friend or relative can contact us on your behalf. We will respond quickly and treat your enquiry confidentially.
Contact our Community Care Intake Officer on 03 92096592. Once you've made contact the Community Care Intake Officer will assess your service eligibility and arrange for an Assessing staff member to help you develop an individual care plan that specifically meets your needs. This usually involves a home visit.
Information and referral to other service providers can also be given at this time.
Cost of Services
There is no cost for the initial assessment. The cost of community care services is based on your household income and a fee is discussed at the time of assessment.

Home Support Services
Home care
Community Care staff can provide help with household duties and chores, which may include vacuuming, mopping and cleaning. Home Care is provided on weekdays during normal working hours.

Shopping Assistance and Bill Paying
This service enables you to access your local precinct and community. One of our staff will drive you to a local shopping precinct so you can do your shopping, banking and bill paying. Shopping assistance is provided on weekdays during normal working hours.
Personal care
Provides help and support with the tasks of daily living that are 'personal' in nature. for example, if you are frail or have a disability that prevents you from showering, bathing, dressing, shaving oreating without assistance, a skilled worker will assist with these tasks.

Home and property maintenance
The home and property maintenance service provides assistance in maintaining a safe, secure and healthy home environment. General tasks are undertaken, such as replacing light globes, installing grab rails and ramps, replacing smoke detector batteries and other minor tasks in the home which do not require the skills of a tradesman. Home and Property Maintenance is provided on weekdays during normal working hours.Delivered Meals
We can deliver fresh and nutritious meals to your home. Both hot and cold meals are available Monday to friday; meals for saturday amd Sunday are delivered chilled on Fridays. Menus cover a seven day week,on a 4week cycle. Clients can choose from three different times throughout the day to receive their meals. Most diet types are catered for, with recommendations from a dietician where required. The menu provides extensive daily choices to suit all tastes and dietary requirements
Community Meals Subsidy
The City of Port Phillip provides susidies for the provision of meals to community groups of elderly people and/or people with disabilities. the subsidies are intended to assist groups whose functions are to encourage people's social opportunities within their community.
Social Meals Drop In
Meals from our delivered meals service are provided lunchtimes on weekdays (excluding public holidays) for those in the community wishing to share a meal in a sociasl setting. These meals are served at the Betty Day Centre on Argyle St.
Assistance for those who care for others
Respite Care
Respite care provides short breaks for those taking care of a family member or friend. Respite Services are not just for carers who live with the person they care for ,but are available for carers with the major responsibility for the well being of a person needing care Care is provided on weekdays during normal working hours but there is some limited capacity for these services to be provided after-hours for clients who have high needs.Carer Support Group
Provides people who care for an older frail person the opportunity to mix with others in a similar caring role, and enables then to seek useful information in relation to caring.

Home Library Services
Regular delivery of library items such as books to those unable to visit the library due to ill-health, frailty or disability. See the Home Library Services page for details.
South Port Day Links (SPDL)
SPDL is a community-based not-for-profit organisation that is managed by an elected Committee of Management. Utilising the skills and enthusiasm of volunteers, SPDL provides a number of services aimed at keeping people in touch with the community and enhancing their quality of life. Their target group is frail elderly people, people with a disability, and those that care for them.

Change Your Clocks - Change Your Smoke Alarm Battery
Your smoke alarm battery should be changed twice a year, once when daylight savings starts and again when it ends.
In the month following the start and end of daylight savings time older residents who already receive property maintenance services from the City of Port PHillip can have their smoke alarm batteries changed as part of their regular service, with a small charge applying to the labour and the cost of the batteries (or smoke alarms).
Older or disabled residents who don't receive property maintenance services can also receive this service, by contacting the council.
Older or disabled Port Phillip residents wanting their smoke alarm batteries changed can use eServices to request the battery change service. Or phone ASSIST on (03) 9209 6777 or TTy (03) 9209 6713 and ask for Community Care.
Older or disabled residents who don't already hava a smoke alarm can purchase them be-forehand either from a shop or via the council's property maintenance service. The subsidised hourly rate is $8.30 per hour.
