ISEPICH is the Inner South East Partnership in Community and Health.  

ISEPICH's vision is:

To improve the health and wellbeing of the inner south east community, through working in partnerships, with a commitment to equity and inclusion.

National Primary Health Care Reform

A New Era in Primary Health Care

Bayside General Practice Network, Monash Division of General Practice and Southcity GP Services have announced that their joint bid with to set up a Medicare Local (ML) has been selected as the preferred application for the Bayside Medicare Local. Subject to some minor further negotiations with the Federal Government, the new Medicare Local is set to commence operations on I January 2012 – less than two months away!

Medicare Locals are primary health care organisations established to coordinate primary health care delivery and tackle local health care needs and service gaps. They will drive improvements in primary health care and ensure that services are better tailored to meet the needs of local communities.

ISEPICH and Kingston Bayside PCPs as well as a number of other primary health care and community organisations were partners in the application and made significant contributions to its content. ISEPICH is a member of the current steering committee and its strategic planning group as well as having a seat on its transition board.

Covering the entire city council areas of Glen Eira, Port Phillip, Stonnington, Kingston and Bayside (plus a little bit of the City of Melbourne) the ML, will in the first instance be managed by a Transition Board consisting of three GPs, two representatives of primary care partnerships, an independent finance expert and a consumer representative. Once the new entity is bedded down, an ongoing Board of Directors will take over governance of the new organisation.

Partnership Plan

ISEPICH regularly produces a plan that sets out the partnership's goals for health promotion, service coordination and chronic disease management.

Partnership Agreement

The ISEPICH Partnership Agreement  sets out the vision, principles, expectations of members and governance structures and processses for the Partnership.

Demographic and Health Status Profile

The Demographic and Health Status Profile contains an overview of relevant demographic and socio-economic data, health status indicators including the Burden of Disease, issues identified by member agencies and needs assessments.
